CT Home Care Program for Elders (CHCPE)

Home Care Solutions for Seniors 65 and Up

The Connecticut Home Care Program for Elders (CHCPE), overseen by the Department of Social Services (DSS), offers a valuable alternative for individuals aged 65+ to live comfortably at home, receiving necessary services and support, rather than moving into a nursing facility or similar institution. CHCPE categorizes its services to align with both the financial means and the care needs of each individual.

Eligibility Requirements

For eligibility, applicants must reside in Connecticut, be 65 years of age or older, and be considered at risk of requiring nursing home care, or already qualify for such care. Financial eligibility is also a criterion, assessing each applicant’s income and assets independently (spousal income is excluded) to determine qualification for the CHCPE benefits. Risk factors include needing assistance with essential activities of daily living such as bathing, dressing, eating, mobility, toileting, and managing medications. Details on the financial criteria for CHCPE can be found here.

Available Services and Support through CHCPE

The CHCPE provides a range of services, including:

  • Care coordination and transition management

  • Bill payment assistance

  • Support brokerage

  • Programs for managing chronic diseases

  • Assistive technology and modifications to the home for accessibilityI
